Sapphire scans the chart, proposes the right APCM tier from chronic-condition count and dual status, and packages consent and care-plan steps into one sitting.

Comprehensive regular assessments

Periodic check-ins with the patient about BP, weight, symptoms, medication lists and barriers, social needs screening — customized to the patient's needs.

Documented monthly summary

At month-end, each enrolled patient has a record with the right APCM tier, the corresponding code, and the documentation needed to submit and defend.
